Switched from semaglutide to tirzepatide — direct comparison

Ran 4 months on semaglutide, then switched to tirzepatide for another 4. Side-by-side notes on weight loss, side effects, and quality of life.

Quick context: 34F, started both peptides for body recomp, not diabetes. Worked with my GP throughout.

Semaglutide first 4 months: ~9% body weight loss, consistent appetite suppression, moderate nausea in weeks 1–3, plateaued by month 4.

Switched to tirzepatide at equivalent starting dose. First month felt different — less nausea but more constipation. Energy was noticeably better than on semaglutide.

Months 5–8 on tirzepatide: additional 7% weight loss with less hunger and better gym performance. The dual mechanism really does feel different — not just stronger.

Honest take: if I were starting over, I'd start with tirzepatide directly if access and cost allowed. The progression felt smoother.

Matches what I see in my cohort — patients who switch often describe tirzepatide as 'cleaner' even when total weight loss is comparable.

Constipation difference is real. Magnesium and fiber audit helped me a lot when I switched.
